What triggers it
Video (non-medical) of people living or dead showing dismemberment, visible innards, burning or charred bodies, or throat-slitting; live-streamed executions; captions or voiceover that express joy at the suffering shown in warning-screen-level imagery of people, or explicit joy at animals' suffering (sadistic remarks).

How to fix it
Cut the graphic segment completely and remove mocking or celebratory commentary.
Good to know
Blur that leaves the injury recognizable does not help. Also check the caption tone: 'he deserved it lol' over violent footage is a sadistic remark. Medical-context videos and still images of these get a warning screen and 18+ instead.
Can FYProof spot it?
A full check of your video looks for it in what's shown, what's said and the caption. What a check finds is an estimate, not a guarantee.
