What triggers it

Coarse language or profanity directed at an individual or group, even if bleeped or blurred.

What Snapchat says

“these Recommendation Eligibility Guidelines prohibit coarse language or profanity directed at an individual or group, even if it is bleeped or obscured”

How to fix it

Cut or reword insults aimed at people.

Good to know

Profanity with a target ('you're a f***ing idiot', insults at a named group). General swearing not aimed at anyone ('this is f***ing amazing') is allowed. Bleeping does not fix targeted profanity.
