What triggers it
Attacks on people for a protected attribute (race, ethnicity, national origin, religion, caste, sex, gender, gender identity, sexual orientation, disability, serious disease, immigration status, tribe): slurs, dehumanizing comparisons, calling a group criminals or diseased, supremacy claims, hateful conspiracies, hate symbols, atrocity denial, deadnaming or misgendering, promoting conversion therapy, selling hate merchandise. In some regions also insulting religion or desecrating religious symbols.

How to fix it
Remove slurs, symbols and generalizations about the group. If you are quoting hate to criticize it, say so clearly on screen and in the caption.
Good to know
Look for slurs (spoken, written, in lyrics of added audio), hate symbols in frame (flags, tattoos, clothing), 'X are animals/criminals' style generalizations, Holocaust or genocide denial, deliberate misgendering of a named person. Does NOT count: in-group reclaiming of slurs in non-degrading ways, discussion of public policy without attacks, documentary/education about hate, fiction that does not promote hate, criticism or satire of hateful ideologies.
