What triggers it
Graphic violence that has adequate context, or dramatized violence with graphic scenes (torture, mass violence against non-combatants, dismemberment, decapitation, violent death with blood), or content that only shows the goriest part of a film or game.
What YouTube says
“We may age-restrict dramatized violence when it contains graphic scenes, such as mass violence against non-combatants or people being tortured, dismembered, or decapitated.”

How to fix it
Shorten or blur the most graphic moments and avoid gore-only compilations.
Good to know
Check whether violent or gory imagery is the focus (e.g. a compilation of only the most violent scenes) and whether real actors are used; real-actor violence is treated more strictly than animation or games.
