What triggers it
Near nudity (nudity covered only by an overlay, opaque object or see-through clothing), shots where crotch, buttocks or breasts are the focus, sexual activity shown only as shapes or silhouettes, recognized fictional implicit sex scenes, kissing with visible tongue, erotic or sexual dancing, simulated sex, sex-act hand gestures, porn-site logos/screenshots/clips, any audio of sexual activity, and sexually suggestive language or sexual metaphors (incl. in jokes).

How to fix it
Reframe or cut the focus shots, swap the sexual audio, remove adult-site logos, and tone down sexual wording if you want teen and non-follower reach.
Good to know
Watch for lingering zooms on body parts, twerking or grinding choreography, moaning sounds (including trending sounds), and watermarks or logos of adult sites (e.g. OnlyFans-style or porn tube logos). Swimwear or gym wear in a normal context does not count by itself. Content that is 18+ is also not recommended and hidden from teens.
